A proclamation was read Tuesday, Aug. 4, and another will be ready by Flagler County, at 5 p.m. Monday, Aug. 17.

For more information, and to get a nomination form, email [email protected].

The following is taken from the nomination form for the awards:

Flagler County is the home of many outstanding elders who have served and inspired others in this community: teachers, nurses, writers, merchants, builders, physicians, historians, environmentalists, healers, journalists, musicians, volunteers, and so many others.  They serve as role models and mentors with their examples of hard work, good deeds, and wisdom.

Anyone may nominate an individual by sending in their choice on this form.  Nominations of exceptional Flagler/Palm Coast men and women, 70 or older, are welcome.  Nominees may have contributed through volunteering or their vocation. In considering nominees, think about the following categories of dedication and contribution: health care, education, community building (peace and justice, homeless causes, elder support, etc.), thriving through adversity, the environment, etc.

After all the nominees are carefully considered, three will be selected for the ceremony. We will notify those chosen for the honor.

We will be expressing our appreciation for them at a simple public ceremony on August 21, National Senior Citizens Day.  Three (3) honorees will be selected from nomination submissions from the Flagler County community. The ceremony will take place at Florida Hospital Flagler, 60 Memorial Medical Pkwy, Palm Coast, in Education Room D from 4 -6 PM.

Submit your nominee form to:  [email protected]

Please give the nominee’s age and tell why (s)he merits the award.  The nomination should describe the person’s community involvement, with examples of leadership, dedication, and specific endeavors. You are encouraged to include personal stories and memories, and how this person affected your life.   Biographical information and any printed articles are helpful. Use another page if needed.

Deadline for submission: Aug. 10, 2015
