Hospital reaches out to community
Jill Piazza, of Ormond Beach, has been selected to serve as the vice president of care integration for the five Florida Hospitals in Volusia and Flagler counties.
The hospital system has been shifting from an “admission to discharge” system of care to “cradle to grave” according to a spokeswoman. Examples of the initiative include the health plan Florida Hospital Care Advantage, the care management program Florida Hospital Community Care and outpatient facilities such as Florida Hospital CentraCare.
Piazza will be responsible for care management, continuum of care integration, and community health needs. She will ensure the various parts of the system become connected and work together.
Piazza, a physical therapist, has been with the Florida Hospital organization for nearly 13 years and most recently served as the assistant vice president of health promotion and wellness.
Piazza is a board member of the United Way of Volusia/Flagler, One Voice for Volusia, and the Fun Coast Worksite Wellness Council.
